Fault Card

A complete Fault Card for a slow AM-650 hoist, what each of its sections captures, and how to build one from handwritten shift logs with the InSkill Field Card Creator skill.

A Fault Card captures what the field knows about one fault on one machine type: the symptoms Technicians report, the checks that found the cause, in the order the causes were actually seen, and what to do when the fix doesn't work. It merges the records from every unit into one card, with pictures. For how Fault Cards fit with the other card types, see Field Cards.

This page shows a complete Fault Card, explains what each section captures, and then shows how to create one with the InSkill Field Card Creator skill.

What Each Section Covers

The card follows the same order every time, so a Technician always knows where to look. Sections 1 to 9 are numbered on the card.

SectionWhat it capturesIn this example
HeaderThe customer logo, the card type, and the card ID with its language suffix. Chips show the status, language, version, review date and, for demo content, a Demo content label.AZ Machine Works logo, Fault card, ACW-AZMachineWorks-AM650-Truck-F-001-EN.
Summary boxEquipment, model years, subsystem and estimated time, plus When to use: the symptoms and alarm codes a Technician reports. Source history says how many records the card rests on and what they found. When to use lists a raise over 11 s, a body that won't raise, AM-4018, AM-4030 and the CD-7 Hoist Locked message. Source history: 8 complaints on MY2025 and MY2022 trucks.
1 SymptomsWhat the Technician sees, hears or reads on the display, with the field values that were recorded and the maker's value to compare against.Raise times of 17 to 21 s from the records, against the manual's 11 ± 1 s.
2 Safety and preconditionsThe hazards of this job and the conditions to set before starting, including lessons learned in the field. Always first among the action sections.Never go under a raised body unless it is empty and on the prop pin; the ML screw limits (½ turn, 1½ turns at most, ¼ turn loaded); never set the hoist relief above 2,500 psi.
3 Tools and partsTest kits, software and parts, with part numbers and notes such as model-year differences.Pressure test kit 650-9210-00, ServiceLink, flow meter kit 650-9125-00, EHL boot 650-4216-00, hoist pump 650-4305-00 (2025 trucks only).
4 Diagnosis: check in this orderThe core expertise: a table of checks, each with the cause it found, the action that fixed it, and Seen, the number of records with that cause. The most frequent cause comes first, so Technicians check the likely thing before the expensive thing.ML screw left open after a manual lowering (seen 3), the park-brake interlock (2), water in the EHL connector (2), a worn hoist pump (1).
5 VerificationHow to prove the fault is gone before the machine goes back to work.Raise in 11 ± 1 s and lower in 13 ± 1 s; no active AM-4018 or AM-4030; clear inactive codes only after a full haul cycle.
6 If not resolvedA workaround that keeps production going, where to escalate and what data to bring, and the related cards and Tasks.Lower the body with the ML screw and park the truck; spool and network checks; escalate to AMW Field Service with codes, raise time and gauge readings.
7 FiguresPictures of what to look for, each with the records it came from. Steps and checks point to them by number.Six illustrations: the ML screw, the Hoist Locked screen, a wet EHL connector, the test panel, metal flakes in a filter housing, and a location view.
8 Field notesThe evidence: a dated, factual record of what happened in the field, with the model year and source IDs. No interpretation.Ten notes from 2026-07-05 to 2026-09-08 on MY2025 and MY2022 trucks.
9 To be confirmedEvery gap that stops the card from becoming OEM verified: values not in the manual, missing part numbers, estimates.Values for MY2022 trucks to check against the 2022 manuals; the oil analysis result; the estimated time.
Legend, keywords and sourcesThe value-tag legend, the keywords InSkill GPT uses to find the card, including shop-floor slang, and the records and manual pages behind the card. The footer repeats the card ID and reads Powered by InSkill. Keywords include "hoist dead", "yellow screw" and "dump lever" alongside the fault codes.

Example Questions

These questions were asked in Ask on Copilot (Truck) with the card uploaded, and InSkill GPT answered them correctly. Use them to check the card after you upload it.

Ask thisA good answer covers
"What raise time is normal for the AM-650 hoist?"Raise from body down to kick-out in 11 ± 1 s and lower in FLOAT in about 13 s, at high idle.
"Raise is still slow, the ML screw is closed and there are no codes. What next, and what do I send AMW if I can't fix it?"TP-2 150–250 psi in HOLD at high idle, raise solenoid 700–750 mA, and pump flow of at least 66 gpm at 2,100 rpm and 1,000 psi. TP-2 above 400 psi means the spool isn't centering. Send AMW Field Service the codes, raise time, TP-2 and TP-6 readings, solenoid current, flow, unit and model year.
"What flow should the hoist pump give on the AM-650?"At least 66 gpm at 2,100 rpm and 1,000 psi, and about 73 gpm after a pump replacement.

How to Create a Fault Card

Fault Cards are created in Claude with the InSkill Field Card Creator skill. You give it the source records, and it reads them, counts how often each cause was found, writes the card, and checks it before handing it over.

Follow these five stages in order:

  1. Gather the source data
  2. Run the skill
  3. Review the card
  4. Upload to InSkill
  5. Test the card in Ask

Gather the source data

Use the records Technicians already keep: shift logs, work orders, maintenance notes and photos, in any language, typed or handwritten. Add the maker's manuals so the skill can tag values it can confirm.

The source for this example is a handwritten shift log page:

A handwritten AZ Copper Works shift log page for the AM-650 truck shop with five hoist entries from July 2026, stamped Demo data.

Handwritten shift log page (fictional demo data)

The skill reads the handwriting into text and gives every entry a source ID, so each statement on the card can be traced back:

Handwritten entrySource IDUsed for
7/5 N B43558 DR: engine stalled, body let down with the ML screwE0001Field notes; the cause of the next entry
7/6 D B43558 JM: hoist slow, 19 s, AM-4018, ML screw still openE0002Diagnosis check 1, symptoms, field notes
7/14 D B43558 RT: "hoist dead", Hoist Locked - Park BrakeE0006Diagnosis check 2
7/17 D B43558 AL: storm, AM-4030, water in the EHL plug, 5V = 5.02E0008Diagnosis check 3
7/22 D B43113 DR: hoist slow, 21 s, ML screw ½ turn openE0010Diagnosis check 1

The rest of the records came from the typed field log for the same period:

id,date,shift,unit,tech,area,entry
E0015,2026-08-03,Night,B43113,DR,Hoist,"Hoist Locked - Truck Moving at Dump 1. Truck creeping on the dump grade. Operator stopped fully and applied the park brake; body raised normally. No fault."
E0024,2026-08-26,Night,B43558,DR,Hoist,"Body lowered with the ML screw during an electrical repair with the engine off. Next shift: slow raise and AM-4018. ML screw open. Closed and torqued lock nut 18 lb-ft. Added 'ML screw closed and torqued' to the shop sign-off sheet."
E0025,2026-08-27,Day,B43113,BS,Hoist,"AM-4030 after overnight rain, lever ignored. EHL boot torn; water in the EHL connector. Replaced the boot, dried the connector, EHL calibration done. Cleared after a full haul cycle."
E0028,2026-09-04,Day,B43558,AL,Hydraulic,"Scheduled return filter change. Bronze-colored flakes in the bottom of the right housing. Reported to supervisor. Oil sample taken. Bypass indicators not reset after the change."
E0029,2026-09-08,Day,B43558,JM,Hoist,"Hoist slow, raise time 17 s, AM-4018 inactive. ML screw closed. TP-2 210 psi in HOLD. Raise solenoid 730 mA at full RAISE. Flow meter at pump: 52 gpm at 2,100 rpm and 1,000 psi. Replaced hoist pump 650-4305-00. Flow 73 gpm. Relief 2,500 psi. TP-6 450 psi. EHL calibration done. Raise 11 s, lower 13 s."

The manuals used were the AMW-SM-650-25 Service Manual (hoist diagnosis on page 33, fault codes on page 39) and the AMW-MP-650-25 Maintenance and Parts Manual (ML screw and prop pin on page 13, flow meter kit on page 52).

Run the skill

In Claude, attach the source files, the manuals and the customer logo, and use a prompt like this:

Use the InSkill Field Card Creator skill.

Create a fault card for "Hoist slow or will not raise" on the AMW AM-650 haul truck.
- Copilot: Copilot (Truck)
- Site: AZ Copper Works (prefix ACW)
- Keep the card generic to the AM-650: no unit numbers; note model-year differences
- Source: the attached shift log page and field log, July to September 2026
- Check every value against the attached AMW-SM-650-25 Service Manual and
AMW-MP-650-25 Maintenance and Parts Manual, and cite the page
- Header: the attached AZ Machine Works logo
- Order the diagnosis by how often each cause appears in the records

The skill works out the site, period, equipment and language from the sources. It asks only for what it can't find, such as the customer logo or which fault to cover. If a fault needs more than about eight steps with settings, measurements or branching, the skill recommends a full Task instead.

Review the card

Before you upload the card, check:

  • Every page reads cleanly, no heading is stranded at the bottom of a page, and the card ID fits the header.
  • Every field value appears in the source records, and every source ID on the card exists.
  • Every value taken from the manual cites its page.
  • The To be confirmed list names every gap, and who should close it.
  • The settings file has InSkill GPT Answers, App and Web all on.

Upload to InSkill

Field cards are uploaded with InSkill GPT Answers, App and Web all on. With App and Web off, InSkill GPT still uses a card, but doesn't show it as a resource in the answer.

  1. Open the copilot, here Copilot (Truck), and open its Resources tab.
  2. Click Bulk Upload and add the PDF.
  3. Under Configure defaults for uploaded files, leave Published on. Under Visibility, leave App, Web and InSkill GPT Answers selected. Under InSkill GPT, leave Searchable and Parse selected.
  4. Click Create 1 Resources, then Done.
  5. Open the new resource and set Name, Keywords and Description from the settings file. Leave Prompt empty. Click Save. See Editing Resource Attributes.
📘

Note

Claude can do step 5 for you through the InSkill MCP Server: ask it to apply the .inskill.json settings file to the uploaded resource.

In this example the resource name is Field Card (Fault) | Hoist slow or will not raise.

Test the card in Ask

Ask the example questions in Ask on the copilot and check the answers against the card.

Output Reference

Files

The skill writes two files for each card:

FileWhat it is
ACW-AZMachineWorks-AM650-Truck-F-001-EN.pdfThe card: a branded, US Letter PDF. This is the file you upload to InSkill.
ACW-AZMachineWorks-AM650-Truck-F-001-EN.inskill.jsonThe settings file: the resource name, keywords, description and visibility to apply in InSkill.

The skill writes the card in English, even when the source records are in another language. It writes another language version, for example ACW-AZMachineWorks-AM650-Truck-F-001-ES.pdf, only when you ask for one. On request, it also delivers a kit with the template, builder, content files and images, so the card can be rebuilt after edits.

Card ID

Every card ID follows the pattern <Site>-<Brand>-<Model>-<MachineType>-<Code>-<Number>, and each language version adds a suffix:

PartIn this exampleMeaning
SiteACWAZ Copper Works, the site the records come from
BrandAZMachineWorksThe machine maker. Process is used when the maker can't be confirmed from the sources.
ModelAM650The machine model. Left out when it doesn't apply.
Machine typeTruckThe machine type
CodeFFault card
Number001The card number for this site, maker, model, machine type and code
Language-ENAdded to the file names. English is the base language.

Value tags

Every value on the card is tagged so Technicians and InSkill GPT know how far to trust it:

TagHow it looks on the cardWhat it means
Field valueGrey value in a dashed boxTaken from the source records. Not yet confirmed by the maker.
Verified valueWhite value on solid blueTaken from the maker's manual, or confirmed by the maker.
To confirmAmber labelAdded by the author, not found in the sources. Validate it before anyone relies on it.

Status, version and review

ItemIn this exampleRule
StatusField-derivedA new card is always Field-derived. It becomes OEM verified once the maker confirms its values and the To be confirmed list is closed.
Version0.1The first version is 0.1.
Review date2027-01-29No more than 90 days after the card is made.
OwnerTo be assignedThe person who keeps the card current.
